Others don't notice clear changes until week three or four of induction. Both timelines are normal, and both are part of how the medication actually works. Here's a realistic look at the timeline we see in our Denver clinic.
Phase 1 — induction
Induction is twice a week for four weeks. Eight sessions total. This is when the brain is doing the most active rewiring, and it's when the bulk of the response builds. By the end of week four, your provider should be able to give you a clear sense of whether Spravato is working for you.
Phase 2 — maintenance
Starting in week five, you typically move to weekly dosing for a month. After that, the cadence stretches — every two weeks, then once a month, then every six weeks for many patients. Maintenance is personalized; it depends on how you're feeling and how durable your gains are between sessions.
What "response" actually looks like
Response isn't always euphoria. The most common pattern is a quiet lifting of the emotional weight — you sleep better, you notice your morning isn't as hard, you stop avoiding things, you laugh at something. Patients often realize they've improved when their partner or coworker mentions it before they do.
The most common timelines we see
Roughly a quarter of our patients feel a clear shift within 24 to 72 hours of their first dose. Another half develop response over weeks two through four. About a quarter of those who eventually respond don't see it until later — sometimes into early maintenance. All of these patterns are normal.
If you don't respond by week four
Week four is a clinical decision point, not a verdict. We review the case, look at whether dose adjustments are appropriate, consider other contributing factors (medical issues, substance use, ongoing trauma without therapy support), and discuss next steps with you. Some patients add therapy. Some try a different ketamine modality. Almost no one is left without a path forward.
How long maintenance usually lasts
This varies. Some patients are in maintenance for six months, others for several years. The goal isn't a fixed endpoint — it's the longest interval between doses at which you feel like yourself. We adjust based on how you're doing, not a calendar.
