Spravato for suicidal ideation is one of the most important developments in psychiatry in the past decade. In 2020, the FDA approved Spravato specifically for adults with major depressive disorder experiencing acute suicidal ideation or behavior — the first medication ever approved with that indication. Here's what the research actually shows, what the approval means, and what your options are if you're in this position. 

The FDA approval for MDSI 

The indication is technically called "major depressive disorder with acute suicidal ideation or behavior" (MDSI). It's separate from the treatment-resistant depression indication. The bar for the MDSI approval was different — patients didn't need to have failed prior medications. They needed to be experiencing active suicidal ideation in addition to major depression. 

What the ASPIRE trials showed 

Two large clinical trials (ASPIRE I and ASPIRE II) studied Spravato alongside standard care in patients with MDSI. Within 24 hours of the first dose, patients on Spravato showed a statistically significant reduction in depression severity compared to placebo. The reduction in suicidal thoughts was less statistically clear-cut, but depression itself improved faster than with standard care alone — a meaningful difference when someone is in acute crisis. 

How quickly can it work? 

Many patients report a meaningful change in mood within hours of the first dose. This is one of the reasons Spravato received the MDSI approval — most antidepressants take weeks to work, which is a long time when someone is in acute crisis. 

How it differs from standard antidepressants in a crisis 

Standard antidepressants work slowly. In an acute suicidal crisis, "we'll wait six weeks and see" isn't always a safe plan. Spravato offers a tool that can act in hours alongside whatever other support the patient is receiving — hospitalization, intensive outpatient care, therapy, family support.
